In this vital role, you will provide personalized care that promotes independence, safety, and well-being in home and community settings. Your energetic approach and attention to detail will help create a supportive environment for a senior couple with cognitiv/memory impairments. This paid position offers an opportunity to foster trust and build lasting relationships while delivering essential care services. Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patients' health status by observing vital signs and overall condition; report changes promptly Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Support clients with medication management and administration following care plans and HIPAA regulations Use specialized equipment (I.e mobility assist devices) Maintain a clean and safe environment through light housekeeping, laundry, and sanitation tasks Document care activities accurately in compliance with established protocols and report back to responsible party Requirements Proven experience in home & community care, assisted living, nursing homes, or group homes is highly preferred Knowledge of dementia care, Alzheimer's care, memory care, hospice care, or working with individuals with developmental disabilities or autism is advantageous Certifications such as CPR (Cardiopulmonary Resuscitation), First Aid, and HIPAA compliance are required or highly recommended Ability to lift heavy objects safely and perform physical tasks involved in patient transfer and mobility support Strong observational skills for patient monitoring and reporting; excellent communication abilities for effective teamwork Valid driver's license and reliable transportation are often necessary for community-based assignments Compassionate attitude with a commitment to providing respectful, person-centered care Round the clock care needed.
