As an independently owned RCFE, we have the flexibility to focus on what truly matters by tailoring care and services to the unique needs of each resident through thoughtful, individualized support that promotes wellness, recovery, dignity, and the highest quality of life. As the Lead Caregiver/ Team Supervisor , you'll provide hands-on caregiving to residents and supervise a team of ~12 caregivers. This role oversees and coaches caregivers on providing exceptional care through constant and direct resident observation, communicating changes with the administrator, family, and practitioners, and proactively taking action to support resident's health and quality of life. This is a full-time, hourly position working on-site Mon-Fri, with a start time around 6am - 7am (on-call nights and weekends for caregiver questions). Responsibilities Provide direct caregiving support to residents with dementia, on hospice, and behavioral/physical challenges, including showering, grooming, dressing, medication management, etc. Visually assess changes in resident conditions throughout the shift and take immediate action to adjust care. Supervisor caregivers and coordinate care among the team to support residents' health, safety, dignity, and wellbeing. Evaluate changes in resident's physical, cognitive, or emotional condition and communicate updates to administrator, clinicians, and family members. Oversee Med Techs to ensure medication is administered as prescribed. Monitor residents for medication effectiveness, side effects, or adverse reactions. Coordinate care needs with physicians, nurses, and therapists. Ensure individualized care plans are followed and updated as resident needs change. Maintain accurate documentation of resident care, observations, and communications. Serve as a primary point of contact for families regarding day-to-day care updates. Supervision Directly supervise and support 12-15 caregivers assigned within the residential home. Provide day-to-day leadership, training, and support to staff. Monitor performance, professionalism, and adherence to care standards. Conduct shift oversight, rounding, and quality assurance checks. Support staffing coordination to ensure appropriate caregiver coverage. Address performance concerns. Assist with caregiver onboarding, training reinforcement, and skill development. Required Qualifications Bachelor's Degree; or 4+ years of relevant work experience 3+ years of direct caregiving experience in a dynamic and highly-active senior-living facility. Lead or supervisor experience preferred Bilingual Spanish preferred Must pass LiveScan criminal background check Strong observational skills with the ability to recognize changes in health conditions and take immediate and appropriate action Good written and verbal communication skills in English Deep desire to serve the senior residents Benefits 401k plan Medical insurance (Kaiser)
