Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs, medication administration, and overall condition updates Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Support clients with mobility using proper patient transfer methods and equipment such as Hoyer lifts or manual handling techniques Maintain detailed care plans and document all patient interactions, observations, and changes in condition in compliance with HIPAA regulations Provide companionship and emotional support to enhance mental well-being, especially for those with dementia or Alzheimer's disease Ensure a clean, safe living environment through regular cleaning, laundry, and household chores Skills Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care settings such as nursing homes or assisted living facilities Strong patient observation skills to detect changes in health status or behavior patterns Knowledge of medication administration, vital signs monitoring, and first aid/CPR certification Familiarity with specialized care such as memory care, hospice care, or caring for individuals with autism or developmental disabilities Ability to assist with mobility aids like Hoyer lifts and perform manual handling safely Excellent communication skills to effectively coordinate with clients' families and healthcare teams Experience working within home health environments while adhering to HIPAA standards for confidentiality This role is ideal for compassionate individuals eager to provide high-quality home care that promotes independence and enhances quality of life.
