Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Monitor patients' health status by observing vital signs, patient condition, and behavioral changes; report findings accurately to healthcare teams. Support medication administration following care plans and ensuring compliance with HIPAA regulations for patient privacy. Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking. Facilitate safe patient transfers using manual handling techniques and equipment such as Hoyer lifts; assist with mobility and positioning. Maintain cleanliness of the home environment through light housekeeping tasks including cleaning, laundry, and organizing personal spaces. Provide transportation assistance for appointments, errands, or community activities while ensuring client safety during travel. Document care activities meticulously in accordance with care plans and communicate effectively with clients and their families about their needs and progress. Skills Proven experience working in home health, assisted living facilities, nursing homes, or group homes is highly desirable. Strong patient observation skills to detect changes in health or behavior promptly. Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and neurodevelopmental disorders such as autism. Ability to perform patient monitoring including vital signs measurement and medication administration. Familiarity with HIPAA regulations to ensure client confidentiality at all times. Skilled in assisting with ADLs and providing personal care support with compassion and professionalism. Experience using patient transfer methods like Hoyer lifts and managing manual handling safely. Capable of heavy lifting when necessary while maintaining proper body mechanics to prevent injury. Excellent communication skills for effective client interaction and teamwork within multidisciplinary settings.
