Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Monitor patients' health by observing vital signs, managing medication administration, and reporting any changes or concerns promptly. Support clients with mobility and transfers using appropriate techniques such as Hoyer lifts or patient transfer methods to ensure safety for both caregiver and client. Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation to promote healthy living. Maintain accurate care plans, document patient progress, and adhere to HIPAA regulations to protect client confidentiality. Provide transportation assistance for medical appointments, errands, or social activities to foster community engagement. Perform light housekeeping duties including cleaning, laundry, and maintaining a safe environment that complies with health standards. Skills Proven experience working with individuals with developmental disabilities, neurodevelopmental disorders, or aging populations such as seniors or those with dementia or Alzheimer's disease. Strong knowledge of patient observation techniques, dementia care, memory care, hospice care, and resident's rights. Familiarity with home health practices including patient monitoring, medication management, vital signs measurement, and first aid/CPR certification. Ability to operate assistive devices like Hoyer lifts safely and effectively while demonstrating manual handling skills. Excellent communication skills for engaging with clients empathetically and collaborating effectively with families and healthcare teams. Experience working in group homes, assisted living facilities, nursing homes, or home & community care environments is highly desirable. Knowledge of HIPAA regulations to ensure confidentiality and privacy of all client information.
