Job Summary Established South Denver-area home care agency seeking an experienced Client Care Manager / Care Coordinator to manage client relationships, caregiver performance, and ongoing care coordination. This is not an entry-level position. We are looking for someone with direct in-home senior care experience who can step into the role with minimal training, take ownership of a caseload, and provide strong support to clients, families, and caregivers. The role combines client service, field care coordination, caregiver management, and documentation Responsibilities Manage an assigned caseload of home care clients and serve as a primary point of contact for clients and families. Conduct client assessments, supervisory visits, and follow-up visits, and keep care plans current. Monitor changes in client needs and communicate updates to caregivers and the internal care team. Work closely with scheduling to help match caregivers with client needs and preferences. Supervise, coach, train, and support caregivers, including addressing performance concerns when needed. Handle client complaints, caregiver issues, incidents, and other service concerns through resolution. Maintain timely, accurate documentation in the agency's care management system. Support new client starts, caregiver introductions, incoming calls, and other day-to-day home care operations Requirements Prior home care or senior care experience required Experience supervising, coaching, or managing caregivers Strong customer service skills with clients, families, and staff Strong organization, communication, documentation, and follow-through CNA or other healthcare certification strongly preferred Valid driver's license, reliable transportation, and ability to travel throughout the South Denver metro area

Pay:

$25.00 - $30.00 per hour Expected hours: 40.0 per week
