Job Summary We are seeking a skilled and compassionate Caregiver to provide exceptional support to clients in their homes. The ideal candidate will possess a strong background in patient care, with experience working with diverse populations including seniors, individuals with disabilities, and those requiring specialized memory or dementia care. As a Skilled Caregiver, you will play a vital role in enhancing the quality of life for our clients by delivering personalized assistance with daily activities and health monitoring. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, grooming, dressing, and mobility support. Monitor patients' health status by observing vital signs and reporting any changes to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation. Support clients with medication administration following prescribed protocols while maintaining HIPAA compliance. Operate patient transfer equipment such as Hoyer lifts safely to prevent injury during mobility assistance. Maintain a clean and safe environment by performing light housekeeping tasks and adhering to infection control standards. Provide transportation assistance for appointments, errands, or community outings as needed. Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Strong patient observation skills to identify changes in condition or behavior promptly. Knowledge of dementia care, Alzheimer's care, memory care, and neurodevelopmental disorders support. Familiarity with care plans, medication management, vital signs monitoring, and resident's rights. Ability to perform manual handling tasks including heavy lifting and patient transfers using proper techniques. Excellent communication skills for effective client interaction and documentation of care activities. Certification in CPR and first aid; experience with HIPAA regulations is highly desirable.
