Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor patients' vital signs and observe for any changes in health or behavior, documenting findings accurately in compliance with HIPAA regulations Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation to promote healthy living Administer medications safely following prescribed care plans and document medication administration diligently Support clients with cognitive impairments such as dementia or Alzheimer's by implementing memory care strategies and engaging them in meaningful activities Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to ensure overall well-being and comfort Provide emotional support and companionship, fostering social interaction and engagement to improve quality of life Requirements Prior experience in home & community care, assisted living, nursing homes, or group homes is highly desirable Certification in First Aid and CPR is required; additional certifications such as Hoyer lift training or hospice care are a plus Knowledge of patient observation techniques, medication administration, and care planning is essential Ability to work with individuals with developmental disabilities, autism spectrum disorder, dementia, or other special needs Strong communication skills to collaborate effectively with clients, families, and healthcare professionals while maintaining HIPAA compliance Physical ability to perform heavy lifting and assist with mobility support safely Experience working with seniors or individuals with disabilities in long-term care settings is preferred Join us to provide compassionate care that truly makes a difference!
