Duties Assist clients with personal care tasks including bathing, grooming, dressing, and mobility support using appropriate patient transfer methods such as Hoyer lifts or manual handling techniques. Monitor patients' vital signs and observe for changes in health status to ensure timely intervention and communication with healthcare teams. Support medication administration following HIPAA guidelines to maintain confidentiality and safety standards. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation. Provide companionship and emotional support while engaging clients in meaningful activities to promote mental well-being. Maintain accurate care plans and document patient progress, observations, and any incidents or changes in condition diligently. Ensure a clean and safe environment by assisting with light cleaning duties and maintaining infection control protocols. Support residents' rights by respecting privacy, promoting independence, and encouraging participation in community activities or outings with personal transportation assistance when needed. Requirements Proven experience working with individuals with developmental disabilities, neurodevelopmental disorders, dementia, Alzheimer's disease, or in geriatrics is highly preferred. Strong knowledge of patient observation techniques, medication administration, vital signs monitoring, and emergency response procedures including CPR and first aid certification. Familiarity with assisted living environments, nursing homes, group homes, hospice care, home health services, or long-term care settings is advantageous. Ability to perform heavy lifting safely and use patient transfer equipment such as Hoyer lifts when necessary. Excellent communication skills for effective client interaction and collaboration with healthcare teams while maintaining confidentiality under HIPAA regulations. Flexibility to work various shifts including full-time, part-time, per diem or live-in arrangements based on client needs.
