We help seniors and adults live safely and comfortably at home through personal care, companion care, homemaker services, respite care, our Adult Day Center, transportation, and more. We work with private pay, long-term care insurance, VA, Medicaid waiver, and state program clients. The Opportunity We're looking for a compassionate, organized Care Manager to be the main point of contact for our clients and their families. This role starts as part-time. After 90 days, there is potential to move into a full-time position based on performance and agency needs. What You'll Do Meet with prospective clients and families in person, virtually, or by phone to explain our services and payment options. Conduct in-home assessments and build individualized care plans and schedules in our EMR. Introduce caregivers on the first day of service and complete follow-up calls, monthly home visits, and required visit reports. Reassess care plans every 90 days or after a change in condition. Serve as the go-to person for caregivers on client-related issues and ensure visit documentation is complete. Resolve client concerns quickly and document incidents within 24 hours. Coordinate with our Care Coordination team, VA and Access Agency contacts, and referral partners. Provide in-person assistive device training to caregivers. Share after-hours on-call support, including some evenings and weekends. What We're Looking For Experience in home care, care coordination, case management, or a related healthcare setting. Familiarity with Connecticut Medicaid waiver programs, VA benefits, or long-term care insurance is a plus. Comfort with EMR systems, mobile scheduling apps, email, and phone communication. Strong communication skills and a warm, professional approach with families. Valid Connecticut driver's license, reliable transportation, and current auto insurance.
