Care Manager - Home Care HomeWell Care Services of Danbury Danbury, CT Full-Time | $65,000-$72,000 per year + Mileage Reimbursement Job Overview HomeWell Care Services of Danbury is seeking an experienced, dependable, and compassionate Care Manager to oversee the care and overall experience of our clients. This is a hands-on position for someone with professional in-home care or in-home healthcare experience who is comfortable working independently with clients, families, and caregivers. The Care Manager will conduct in-home assessments, develop and update care plans, monitor the quality of care being provided, communicate with families, address client and caregiver concerns, and help ensure each case is running smoothly. The Care Manager will also work closely with our scheduling team and participate in on-call responsibilities, including assisting with caregiver call-outs, last-minute coverage needs, and urgent client or staffing situations. This is not primarily a desk position. The Care Manager will regularly visit clients throughout our service area and should be comfortable independently assessing situations and solving problems in the home. Responsibilities Conduct comprehensive in-home client assessments and reassessments Evaluate client needs including activities of daily living (ADLs), mobility, transfers, fall risk, cognition, home safety, behavioral concerns, and required level of caregiver assistance Develop, review, and update individualized care plans based on each client's needs Maintain regular communication with clients and family members Conduct client visits and follow-ups to monitor satisfaction and quality of care Monitor changes in client needs and determine when changes to the care plan or level of assistance may be necessary Address caregiver performance and client-care concerns Help match caregivers with clients based on care needs, abilities, personality, and client preferences Provide caregivers with clear direction regarding client needs, expectations, and care plans Help coordinate smooth transitions from hospitals, rehabilitation facilities, skilled nursing facilities, assisted living communities, or other settings into home care Communicate and coordinate, when appropriate, with home health agencies, physicians, nurses, therapists, social workers, senior living communities, and other members of the client's care team Work closely with the scheduling team to resolve difficult coverage situations Assist with caregiver call-outs and urgent scheduling needs Participate in the agency's on-call rotation Accurately document assessments, client visits, concerns, changes, and follow-up actions in the agency's electronic system Maintain client confidentiality and follow agency policies, applicable regulations, and documentation requirements Identify problems early and work with the team to develop appropriate solutions Escalate serious client, caregiver, or safety concerns to the Director when appropriate Qualifications Professional in-home care or in-home healthcare experience is required Experience may include home care, home health, visiting nursing, in-home rehabilitation, community-based case management, or another field-based healthcare setting Previous care management, case management, supervisory, or client-service experience preferred Strong understanding of the needs of older adults and individuals requiring assistance in the home Strong communication, assessment, and problem-solving skills Comfortable communicating with clients and families during difficult or stressful situations Able to work independently and make sound decisions in the field Strong organizational and documentation skills Comfortable working with and providing direction to caregivers Comfortable using electronic documentation and scheduling systems Valid driver's license and reliable transportation required Must be willing to travel throughout the agency's service area Must be able to participate in an on-call rotation Candidates from professional backgrounds such as RN, LPN, Social Work, Occupational Therapy/OTA, Physical Therapy/PTA, home health, or experienced home care management are encouraged to apply. A nursing license is not required. Regardless of professional background, previous experience providing or managing services in clients' homes is required. Schedule Full-time position, primarily Monday through Friday. Some evening, weekend, and after-hours availability will be required as part of the agency's on-call rotation. Compensation $65,000-$72,000 per year , depending on experience and qualifications. Mileage reimbursement is provided for approved business-related travel.
