Qualifications:

• High School Diploma/GED Equivalent
• Reliable Personal Transportation
• Maintain & show proof of valid Florida Driver's License to include maintaining & show proof of vehicle insurance
• 3 years or more of experience in providing Senior Care is preferred or within another related field
• Must pass a Level II FDLE criminal background screen and drug screen

SUMMARY:

This position is an Ambassadorship for ECCOA regarding the programs and services offered to our neighbors {seniors} throughout many communities. The role of providing assigned functions in both the home and community is vital to ensure our neighbors, the seniors we serve, remain independent in their homes and their communities. This position may vary in responsibilities as noted below.

Performance Requirements:

• Must be trustworthy to work independently while in the participants' home and with the participant in the community
• Demonstrate an attitude of cooperation {teamwork} and professionalism when working in the home or office
• Be kind, considerate and compassionate with our seniors, team members and all other stakeholders
• Able to be at assigned service location/s on time
• Task oriented while being flexible to the needs of those, we serve
• Must be able to read, write, and follow written or verbal directions
• Must be able to organize work to complete assignments within the required time frame
• Must have good listening and effective communication skills
• Must adhere to confidentiality rules and guidelines to include HIPPA regulations and other required training annually
• Willingness to learn and be responsive to the needs of seniors, their families, team members and other key stakeholders
• Must have a basic understanding of issues relating to seniors and issues faced with aging
• Must have a working phone that has available voicemail and willing to check voicemail and work emails prior to and at the end of each daily work assignment and respond accordingly
• Ability to download the Microsoft Office Teams app to cell phone
• Basic computer knowledge
• Follow guidelines for submitting required Service Logs, Timesheets, travel, and other agency required forms
• Knowledge of principles and processes for providing services to participants, including needs determinants, meeting quality standards and evaluation of client satisfaction
• Attend orientation, in-service training sessions and team meetings

DUTIES/RESPONSIBILITIES

• Homemaking May include but is not limited to the following duties: Complete homemaking tasks, including vacuuming, mopping, cleaning bathrooms, changing bed linens, removal of trash, dusting of furniture, laundry, meal preparation, shopping (when approved) and any other duties as assigned by Lead IHCS team member that are within the scope of service for ECCOA and this position.
• Companion Care May include, but is not limited to the following duties: Provide companionship, supervise the client and/or assist the client in meal preparation, snacks, assisting or supervising client with grooming and dressing, light housekeeping, provide laundry and linen service which may include washing, ironing and folding clothes, provide assistance and planning of clients recreational activities and events such as reading, writing letters, games, stimulating conversation, and any other duties as assigned by Lead IHCS team member that are within the scope of service for ECCOA and this position.

• Respite Care May include but is not limited to the following duties: Provide temporary relief to care givers who are caring for a person with disabilities through the care and supervision of the client, ensuring the safety and well-being of that person, meal preparation, personal care assistance such as dressing and toileting, provide companionship as appropriate, and any other duties as assigned by Lead IHCS team member that are within the scope of service for ECCOA and this position.

RESPONSIBITIES/ACTIVITIES ASSOCIATED WITH ABOVE SERVICES

• Teach/perform meal planning and preparation, routine housekeeping activities such as making/changing beds, dusting, vacuuming, washing floors, cleaning kitchen and bathroom, and laundry.
• Perform/assist with essential shopping/errands.
• Escort to medical facilities, errands, shopping, and outings as specified in the Care Plan.
• Provide companionship, friendship, and emotional support.
• Assist participants with communication by writing or typing correspondence for them or researching information for them.
• Participate on the Care Team by providing input and making suggestions.
• Ensure service is delivered in accordance with all relevant policies, procedures, and practices.
• Monitor supplies and resources.
• Monitor and evaluate services requested and evaluate the progress of the participant and report information to the Case Manager of any changes to include making recommendations to the accessed services and or supplies.
• Follow the written Care Plan and communicate with Case Manager if changes are needed.
• Carry out duties as assigned by the Lead In-Home & Community team member.
• Observe the participant's functioning and report to Lead In Home & Community team member, Case Manager and or the Director.
• Complete and maintain daily Service Logs, monitor participants daily/weekly activities, and document observations as needed.
• Communicate with Lead In Home & Community team member, Case Managers and other ECCOA team members regarding participants status, progress, services and needs.
• Monitor participants supplies and resources.
