Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health by observing vital signs, medication adherence, and overall condition, reporting any concerns promptly Prepare nutritious meals and assist with meal planning to meet dietary requirements and preferences Support patients with mobility using equipment such as Hoyer lifts or other assistive devices to ensure safe transfers Maintain a clean and safe environment by performing light housekeeping tasks like cleaning, laundry, and organizing living spaces Follow care plans meticulously, documenting patient progress and updating records in compliance with HIPAA regulations Provide companionship and emotional support to reduce feelings of isolation, especially for individuals with dementia or developmental disabilities Skills Proven experience in home & community care or assisted living environments Knowledge of patient observation techniques and vital signs monitoring Ability to assist with ADLs and provide specialized dementia or Alzheimer's care Familiarity with medication administration and safety protocols including CPR and First Aid certification Experience working with individuals with developmental disabilities, autism, or memory impairments Strong communication skills for effective interaction with clients, families, and healthcare professionals Physical ability for heavy lifting, assisting with transfers using Hoyer lifts, and long periods of standing or walking Join us in delivering compassionate care that empowers individuals to live comfortably and confidently at home.
