Job Summary:

MUST HAVE CURRENT GROUP HOME EXPERIENCE. HAVE ALL APD REQUIRED DOCUMENTATION.

We are seeking a dedicated and compassionate individual to provide direct support to a client in a wheelchair during daily community outings. This role is scheduled Monday through Friday from 8:00 AM to 2:00 PM. Transportation for outings will be provided via Topps Transport.

Key Responsibilities:

Plan and accompany the client on daily community outings Assist with mobility and personal care, including frequent changing Feed the client lunch while out in the community Accurately chart the client's daily experiences using a computerized logging system Demonstrate strong writing and communication skills for effective documentation and interaction

MUST BE ABLE TO DRIVE TO AND FROM COMMUNITY OUTINGS MUST BE FAMILIAR WITH THE ICONNECT SYSTEM TO WRITE PROGRESS NOTES

Qualifications:

Ability to perform personal care/hygiene tasks with dignity and respect Basic computer literacy and ability to complete digital logs VIA IConnect. Excellent verbal and written communication skills

INCLUDING BUT NOT LIMITED TO

Must have a High School Diploma, or equivalent. Must pass Local and National background checks.

HIPPA, DIRECT CARE CORE COMPENTENCIES, ZERO TOLORANCE, CPT/FIRST AID, RESUME, 4 PERSONAL REFERENCE

LETTERS, have clean driving record for the last 3 years, Have a valid FL. driver's license. Have reliable transportation. Must be eligible to work in the United States with proper documentation.
