Responsibilities Provide comprehensive personal care assistance, including helping residents with activities of daily living (ADLs) Monitor patients' vital signs and observe for changes in health status, documenting findings accurately while adhering to HIPAA regulations to protect patient privacy. Support seniors with meal preparation, feeding, and ensuring nutritional needs are met while maintaining a clean and safe environment through regular cleaning duties. Engage in patient observation to identify any signs of distress or deterioration, especially in individuals with dementia, Alzheimer's, autism, or neurodevelopmental disorders. Facilitate transportation for appointments or community outings using personal transportation assistance while ensuring safety protocols are followed. Maintain accurate records of patient monitoring, care activities, and incident reports in compliance with healthcare standards such as HIPAA. Support hospice or home health services by providing compassionate end-of-life care or ongoing support within the community setting. Experience Prior experience working in assisted living facilities, nursing homes, or long-term care environments is highly preferred. Demonstrated knowledge of dementia care, memory care strategies, Alzheimer's care, and hospice practices. Experience working with individuals with developmental disabilities or neurodevelopmental disorders such as autism is advantageous. Proven ability to perform patient monitoring and assist with ADLs effectively. Familiarity with HIPAA regulations and maintaining confidentiality of sensitive health information is essential.
