Responsibilities Deliver personalized patient care by assisting with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support using patient transfer methods like Hoyer lifts and manual handling techniques Monitor vital signs, observe patient conditions, and report changes promptly to healthcare professionals to ensure optimal health management Prepare nutritious meals aligned with dietary restrictions and assist clients with meal consumption to promote health and comfort Support clients with home & community care needs, including transportation assistance for appointments or errands and facilitating engagement in social activities Maintain detailed documentation of patient observations, care provided, and adherence to resident's rights policies Perform light cleaning duties to uphold a safe and hygienic environment within the home setting Requirements Experience with manual handling and heavy lifting is preferred Certification in CPR and First Aid; ongoing training in patient monitoring and safety protocols is highly valued Strong communication skills to effectively interact with clients, family members, and healthcare teams while respecting residents' rights Familiarity with HIPAA regulations ensuring confidentiality of all patient information Valid driver's license and reliable transportation for client outings or appointments are preferred Join us in providing exceptional home health care that empowers individuals to live comfortably and independently.
