Responsibilities Provide personal care assistance including bathing, grooming, dressing, and mobility support using appropriate patient transfer methods such as Hoyer lifts and manual handling techniques. Assist clients with Activities of Daily Living (ADLs), ensuring their comfort, safety, and dignity at all times. Support medication administration and monitor vital signs in accordance with care plans and HIPAA regulations to ensure patient safety. Engage clients through meaningful conversation, recreational activities, and companionship to promote mental well-being and reduce feelings of loneliness. Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation. Maintain a clean, safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces. Document observations related to patient health, behavior, and progress accurately in compliance with healthcare standards. Support residents' rights by respecting privacy, confidentiality, and individual preferences in all interactions. Assist with transportation needs for appointments or community outings using safe patient transfer techniques. Follow all safety protocols including first aid procedures and CPR when necessary to respond promptly to emergencies. Experience Candidates should bring prior experience working in group homes, assisted living facilities, nursing homes, or home health environments. Experience working with individuals with Alzheimer's disease, dementia, autism spectrum disorder, developmental disabilities or neurodevelopmental disorders is highly valued. Familiarity with care plans, medication administration, patient monitoring—including vital signs—and resident's rights is essential. Previous roles involving hospice care or geriatrics will be advantageous. Demonstrated ability to perform patient transfers safely using Hoyer lifts or other manual handling equipment is preferred. Candidates must have strong communication skills for effective client interaction and documentation. Certification in CPR and first aid is required; additional training in HIPAA compliance or specialized caregiving certifications is a plus.
