Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health status by taking vital signs and observing for any changes or concerns Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Support medication administration following care plans and ensuring compliance with HIPAA regulations for patient privacy Use proper patient transfer methods such as Hoyer lifts and manual handling techniques to ensure safety during movement Maintain detailed documentation of care provided, patient observations, and any incidents in compliance with home health standards Provide personal transportation assistance for appointments, errands, or community activities to promote social engagement Requirements Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care environments such as assisted living or nursing homes Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and patient monitoring techniques Ability to perform light housekeeping tasks including cleaning and maintaining a safe environment for clients Certification in CPR and First Aid is required; additional certifications in manual handling or Hoyer lift operation are a plus Strong communication skills to effectively interact with clients, family members, and healthcare professionals Experience working with individuals with autism or developmental disabilities is highly valued Ability to handle heavy lifting safely and assist with transfers using proper patient transfer methods Join us to provide compassionate home & community care that enhances quality of life.
