Part Time/ Full-Time Position Summary:
The Home Care Field Supervisor is responsible for overseeing the delivery of quality home care services to clients in their homes. This position provides field supervision and support to caregivers, monitors client satisfaction and safety, conducts home visits, reviews care plans, assists with caregiver scheduling and performance management, and ensures compliance with company policies and applicable state and federal regulations. The Field Supervisor serves as an important link between clients, families, caregivers, office staff, and management. The supervisor must be dependable, organized, responsive, and able to identify and resolve problems quickly while maintaining client dignity, privacy, and independence. Essential Duties and Responsibilities 1. Client Care Oversight Conduct initial and ongoing home visits to assess client needs and quality of services being provided. Monitor whether caregivers are following the client's approved care plan. Identify changes in the client's condition, behavior, mobility, or care needs and report them to the appropriate manager or healthcare professional. Ensure clients receive services according to their authorized schedule and care plan. Follow up on client and family concerns and assist in resolving complaints. Monitor client satisfaction and identify opportunities to improve service quality. Promote client dignity, independence, privacy, and respect at all times. Communicate significant changes or concerns to management promptly. 2. Caregiver Supervision Provide field supervision and support to caregivers working in clients' homes. Conduct caregiver supervisory visits and performance observations. Keep current documents/ training up to date. Ensure caregivers understand and follow individual client care plans. Provide coaching, guidance, and corrective feedback when necessary. Identify caregiver training needs and recommend additional training. Monitor caregiver attendance, punctuality, professionalism, and performance. Address caregiver complaints, conflicts, and performance issues. Recognize excellent caregiver performance and encourage professional development. Assist management with disciplinary actions when appropriate. 3. Care Plan Management Review client care plans with caregivers and ensure they understand assigned duties. Communicate approved changes in the care plan to caregivers. Ensure caregivers do not perform tasks outside their authorized scope of duties. Report significant changes in client needs to the appropriate supervisor or clinical personnel. Participate in care-plan reviews when required. Ensure documentation reflects services actually provided. 4. Client and Family Communication Maintain professional communication with clients and their families. Respond to questions and concerns in a timely manner. Explain agency policies and procedures when appropriate. Escalate complaints or serious concerns to management. Maintain confidentiality of client and family information. Build positive relationships with clients, families, caregivers, and referral partners. 5. Incident Management Respond appropriately to reports of falls, injuries, medication concerns, abuse, neglect, emergencies, or other incidents. Immediately report serious incidents according to company policy and applicable regulations. Complete incident reports accurately and within required timeframes. Participate in investigations and follow-up activities. Ensure appropriate corrective actions are communicated and implemented. 6. Documentation and Compliance Maintain accurate and timely records of home visits, caregiver supervision, client concerns, incidents, and follow-up activities. Review caregiver documentation for completeness and accuracy. Protect confidential client information and follow HIPAA/privacy requirements as applicable. Follow agency policies, procedures, and regulatory requirements. Assist with audits, inspections, licensing reviews, and quality assurance activities. Maintain required certifications and training. 9. Staff Training and Development Assist with orientation and onboarding of new caregivers. Reinforce proper client-care techniques and agency procedures. Provide coaching during field visits. Identify recurring performance issues and recommend training solutions. Participate in caregiver meetings and educational programs. Promote professionalism, teamwork, accountability, and excellent customer service. Required Qualifications High school diploma or equivalent required. Previous experience in home care, caregiving, healthcare, social services, or a related field. Previous supervisory or team-lead experience preferred. Knowledge of home care operations and caregiver responsibilities. Strong communication and interpersonal skills. Ability to work independently and make appropriate decisions in the field. Reliable transportation and ability to travel to client homes. Valid driver's license and required insurance, if driving is part of the position. Ability to pass required background checks and screenings. Current CPR/First Aid certification if required by the agency or applicable regulations. Additional clinical credentials may be required depending on the duties assigned and state regulations. Physical and Work Requirements - the position may require the employee to: Travel regularly between client homes and agency locations. Spend extended periods driving or working in the field. Enter private residences and work in a variety of home environments. Stand, walk, bend, and sit for extended periods. Assist with limited client-related activities when within the employee's authorized scope. Respond to urgent staffing or client-care situations.
Pay:
From $19.00 per hour Expected hours: 40.0 per week
