Support patients with mobility and transfers using appropriate techniques and equipment like Hoyer lifts or patient transfer methods to promote safety. Monitor patient health by checking vital signs, observing for changes in condition, and reporting findings accurately to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Maintain cleanliness of living spaces by performing light cleaning tasks and ensuring a safe environment in accordance with health standards. Support medication administration under supervision, ensuring proper documentation while adhering to HIPAA privacy regulations. Provide companionship and emotional support while fostering effective communication with clients and their families. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of patient observation techniques, dementia care (including Alzheimer's care), memory care, and neurodevelopmental disorders such as autism is advantageous. Ability to perform manual handling tasks including heavy lifting, patient transfers using Hoyer lifts, and assisting with ADLs safely. Certification in CPR and First Aid is required; additional certifications like HIPAA compliance or medication administration are a plus. Strong communication skills for effective client interaction and team collaboration. Experience working with individuals with disabilities or developmental disabilities supports success in this role.
