Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene. Support patients with medication administration and monitor vital signs to ensure health stability. Prepare nutritious meals aligned with dietary plans and assist with feeding as needed. Conduct patient observation to identify changes in condition or behavior, especially in individuals with dementia, Alzheimer's, or neurodevelopmental disorders. Utilize proper manual handling techniques and patient transfer methods such as Hoyer lifts to ensure safety during mobility assistance. Maintain detailed care plans, document patient progress, and adhere to HIPAA regulations to protect client confidentiality. Provide transportation assistance for medical appointments or community activities, promoting social engagement and independence. Skills Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care. Strong knowledge of home health practices including hospice care, memory care, and geriatrics. Familiarity with patient monitoring tools such as vital signs measurement and medication administration procedures. Ability to perform patient personal care assistance with compassion and professionalism. Excellent communication skills for effective client interaction and team collaboration. Knowledge of HIPAA standards to ensure privacy and confidentiality of health information. Physical stamina for heavy lifting, manual handling, and assisting with transfers using equipment like Hoyer lifts. Experience working in group homes, assisted living facilities, nursing homes, or home & community care environments is highly desirable.
