Duties Assist clients with activities of daily living (ADLs), including bathing, grooming, dressing, and personal hygiene. Monitor patients' health status by observing vital signs, medication adherence, and overall condition, ensuring adherence to care plans. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Support patients with mobility and transfers using proper techniques such as Hoyer lifts and patient transfer methods to ensure safety. Maintain detailed documentation of patient care activities, observations, and progress in compliance with HIPAA regulations. Administer medications accurately under prescribed protocols and document all medication-related activities. Provide emotional support and companionship, fostering a positive environment that promotes mental health and social engagement. Requirements Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, or senior care is highly preferred. Familiarity with group home settings, assisted living facilities, nursing homes, or home health environments. Knowledge of patient observation techniques, dementia care including Alzheimer's and memory care strategies, and hospice or palliative care practices. Ability to perform manual handling tasks including heavy lifting and patient transfers safely; CPR and first aid certification required or willing to obtain upon hire. Experience working with individuals with neurodevelopmental disorders or autism spectrum disorder is a strong plus. Excellent communication skills for client interaction, family updates, and team collaboration. Ability to follow care plans meticulously while maintaining confidentiality in accordance with HIPAA standards. Valid driver's license for personal transportation assistance if required; flexibility to work various shifts including evenings or weekends as needed.
