Job Overview Join our dedicated home care team and make a meaningful difference in the lives of individuals requiring personalized, compassionate support in their own homes. As a Home Care Provider, you will deliver essential assistance that promotes independence, safety, and well-being for clients across various stages of life and health conditions. This role offers an opportunity to provide vital patient-centered care while fostering trusting relationships and enhancing quality of life. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' health status, including vital signs and overall condition, ensuring timely reporting of any changes Administer medications accurately following prescribed schedules and medical documentation protocols Support patients with medical procedures such as catheter care, wound care, and other specialized treatments Maintain detailed records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring compliance with healthcare regulations like HIPAA Provide companionship and emotional support to clients, especially those with dementia or Alzheimer's disease Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment Use patient transfer methods safely, including Hoyer lifts or other mobility aids, to prevent injury for both client and caregiver Qualifications Proven experience in home healthcare, assisted living, nursing homes, or long-term care environments Strong knowledge of medical terminology, medication administration, and vital signs monitoring Experience working with individuals with developmental disabilities or geriatrics, including dementia and Alzheimer's care Certifications in First Aid, Basic Life Support (BLS), CPR, and hospice or end-of-life care preferred Familiarity with EMR/EHR systems and medical documentation standards Ability to lift heavy objects safely and perform patient transfers using appropriate techniques and equipment Excellent communication skills for client interaction and team collaboration Knowledge of home health care regulations and maintaining patient confidentiality under HIPAA standards Join us to provide compassionate home health care that truly transforms lives!
