Responsibilities:
Assist with activities of daily living: bathing, grooming, dressing, and meal preparation. Monitor and record vital signs and report any changes to the healthcare team. Provide companionship and emotional support to foster a positive environment. Assist with mobility and transfers to ensure client safety. Support memory care clients with reminders and engaging activities. Collaborate with healthcare professionals to implement individualized care plans. Maintain a clean, safe, and organized living environment for clients. Follow all health and safety regulations during care delivery.Qualifications:
Previous experience in home care or healthcare setting required . Familiarity with hospice care is a plus . Ability to monitor vital signs and recognize changes in health status. Strong communication skills (written and verbal) for interaction with clients, families, and healthcare teams. Compassionate, patient, and dedicated to enhancing others' quality of life. Must pass background checks in accordance with state requirements. Licenses /Certifications:
