Responsibilities Provide comprehensive personal care assistance, including help with bathing, grooming, dressing, and mobility support. Support residents with memory care needs, including Alzheimer's and dementia care, ensuring a safe environment. Operate patient transfer methods such as Hoyer lifts and manual handling techniques to ensure safe mobility. Prepare nutritious meals tailored to individual dietary requirements and assist with meal consumption when necessary. Maintain cleanliness of residents' living areas through regular cleaning and sanitation practices in compliance with health standards. Administer medications following prescribed protocols while adhering to HIPAA regulations for patient confidentiality. Support residents' participation in community activities or outings using personal transportation assistance. Document all care activities accurately and communicate effectively with healthcare teams regarding resident status. Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Strong knowledge of patient observation techniques, vital signs monitoring, and medication administration. Expertise in dementia care, Alzheimer's care, memory care, and neurodevelopmental disorders support. Familiarity with HIPAA regulations to ensure confidentiality and privacy of patient information. Ability to perform manual handling tasks safely using equipment like Hoyer lifts; capable of heavy lifting when required. Experience working with individuals with disabilities or developmental/intellectual disabilities support needs. Skilled in creating and following personalized care plans tailored to each resident's needs. Knowledge of resident's rights legislation and ethical caregiving practices. Competence in first aid procedures and CPR certification for emergency response readiness. Excellent communication skills for effective client interaction and team collaboration within home & community care settings.
