Responsibilities Deliver direct personal care assistance, including help with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor patients' health by observing vital signs, assessing symptoms, and reporting changes promptly to healthcare professionals Assist with medication administration following care plans and HIPAA regulations to ensure privacy and safety Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation Support clients with mobility using proper patient transfer methods and equipment such as Hoyer lifts or manual handling techniques Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing living spaces Provide transportation assistance for medical appointments or community outings while ensuring client safety at all times Document care activities accurately in compliance with healthcare standards and communicate effectively with clients' families or healthcare teams Requirements Proven experience working in home health, assisted living, nursing homes, or group homes; experience with dementia or Alzheimer's care is highly valued Knowledge of patient observation techniques, vital signs monitoring, medication administration, and emergency response procedures such as CPR and first aid Familiarity with HIPAA regulations to protect client confidentiality and privacy Ability to perform manual handling tasks safely, including heavy lifting when necessary Strong communication skills for client interaction and team collaboration Experience working with individuals with developmental disabilities, neurodevelopmental disorders, autism spectrum disorder (ASD), or intellectual disabilities support is preferred Valid certifications in CPR and first aid; additional certifications in hospice care or geriatrics are advantageous Reliable transportation and the ability to work flexible hours in a home setting Join us in providing compassionate care that empowers individuals to live healthier, happier lives.
