Support clients with light housekeeping tasks, meal preparation, and transportation to appointments or errands. Follow individualized care plans meticulously, ensuring all client needs are met with professionalism and compassion. Maintain accurate documentation of care provided, including observations, changes in condition, and completed tasks. Collaborate with family members, healthcare professionals, and case managers to ensure comprehensive care delivery. Experience Prior experience in home health care or personal caregiving is highly preferred. Experience working with diverse populations including geriatrics and individuals with chronic conditions is advantageous.
