Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote comfort and independence. Monitor patients' vital signs, observe health status, and report changes promptly to healthcare professionals to ensure optimal care. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking. Support clients with medication administration according to care plans while adhering to HIPAA regulations for patient privacy and confidentiality. Utilize patient transfer methods such as Hoyer lifts or manual handling techniques to ensure safe mobility and prevent injury. Provide transportation assistance for appointments, errands, or community activities to foster social engagement and independence. Maintain detailed documentation of care provided, patient observations, and adherence to care plans while respecting residents' rights and confidentiality standards. Requirements Proven experience working with individuals with disabilities, including developmental or intellectual disabilities support, autism spectrum disorder, or neurodevelopmental disorders. Prior experience in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of dementia care, Alzheimer's care, memory care strategies, hospice care, and geriatrics is beneficial. Certification in CPR and First Aid is required; additional training in patient observation and manual handling is a plus. Ability to lift heavy objects safely and perform patient transfers using appropriate techniques such as Hoyer lifts or other transfer methods. Strong communication skills for effective client interaction and collaboration with healthcare teams while respecting resident's rights. Valid driver's license or reliable transportation for client outings or appointments is preferred. $16.50 - $18.00 per hour Expected hours: 16.0 - 40.0 per week
