Make a Difference Every Day - Join Mastercare Homecare Are you an organized, compassionate healthcare professional who enjoys helping people and making sure things get done? Mastercare Homecare is looking for an experienced Care Coordinator to join our Kailua-Kona team.
Our mission is simple:
to improve the lives of those needing care and companionship in their homes. Responsibilities Coordinate and monitor home care services for assigned members. Communicate regularly with members their families, caregivers, supervisors, and healthcare professionals. Coordinate caregiver schedules and assist with coverage for open shifts, call-outs, and schedule changes. Monitor caregiver attendance, punctuality, and adherence to assigned schedules. Follow up on members and family concerns and work with management to resolve issues promptly. Conduct member and caregiver check-ins and follow up regarding service satisfaction and quality of care. Assist with new member intake, service coordination, and transition to ongoing care. Maintain accurate and timely member, caregiver, scheduling, and service documentation. Monitor service authorization and scheduling information as assigned. Communicate changes in member condition, service needs, or concerns to the appropriate supervisor or clinical team. Coordinate with the RN Supervisor and office management regarding member care needs. Assist with caregiver recruitment, orientation, and onboarding as needed. Ensure caregivers understand member schedules, service expectations, and applicable care instructions. Assist with maintaining compliance with company policies, procedures, and applicable Hawaii home care requirements. Make field visits to members' homes when required. Maintain professional relationships with members their families, caregivers, referral partners, and community organizations. Respond appropriately to urgent scheduling and member service issues. Protect members confidentiality and comply with HIPAA and company privacy policies. Complete other duties as assigned by management. Minimum Qualifications & Requirements High school diploma or equivalent required. At least 1-2 years of experience in home care, healthcare, caregiving, case management, scheduling, or a related field preferred. Experience working with seniors, individuals with disabilities, or other vulnerable populations preferred. Strong organizational and time-management skills. Excellent verbal and written communication skills. Strong customer service and interpersonal skills. Ability to handle sensitive member and employee information with professionalism and confidentiality. Ability to multitask and prioritize responsibilities in a fast-paced environment. Strong attention to detail and follow-through. Comfortable using computers, email, Microsoft Office, scheduling systems, and electronic documentation platforms. Ability to work independently while also functioning effectively as part of a team. Must have reliable transportation and the ability to travel to members homes throughout Maui as required. Valid driver's license and acceptable driving record required. Must successfully complete required background checks and pre-employment screening.
