Please read the full details below to understand the job responsibilities and requirements. --- This is an hourly role for a Personal Support Worker (PSW) . The role is best suited for someone seeking supplemental income who enjoys providing consistent, person-centered support in a home setting. The position is funded by the Illinois Department of Human Services with a set monthly budget, which is approved by the caregiver of the individual. The PSW must submit completed hours into an electronic timesheet and will be paid semi-monthly (every 15th and 30th/31st). Who will you support? Carla is a 56-year-old African American woman who enjoys being active in her community and maintaining her independence. She communicates, expresses her emotions, and is able to participate in many aspects of her daily routine. Carla is working toward greater independence with the long-term goal of being able to live more independently. The PSW will support her in maintaining consistent routines, managing daily living activities, participating in the community, and maintaining a clean and comfortable home. The family is looking for someone who is patient, dependable, observant, organized, and comfortable providing support without taking over tasks that Carla can complete independently. The PSW will become an important source of consistent support in the home, particularly during times when family members are not present.

Here is the desired schedule:

Sunday (2-4PM) Flexible — any time after 2PM for a 2-hour shift Be present for grocery and hygiene-item deliveries Prepare lunches for the upcoming week Prepare dinners for the upcoming week Assist with spot cleaning as needed Tuesday (6-8AM) Prompt and support Carla through her morning routine Monitor PACE transportation and ensure she is picked up on time Contact PACE if transportation is delayed or appears to be going to the wrong address Complete laundry and change linens as needed (bed sheets, blankets, shower curtain, etc.) Wednesday (6-8AM) Prompt and support Carla through her morning routine Monitor PACE transportation and ensure she is picked up on time Contact PACE if transportation is delayed or appears to be going to the wrong address Assist with spot cleaning as needed Thursday (6-8AM) Prompt and support Carla through her morning routine Monitor PACE transportation and ensure she is picked up on time Contact PACE if transportation is delayed or appears to be going to the wrong address Help Carla prepare and pack her travel bag for the weekend Friday (6-7AM) Prompt and support Carla through her morning routine Monitor PACE transportation and ensure she is picked up on time Contact PACE if transportation is delayed or appears to be going to the wrong address Assist with spot cleaning as needed Additional Support Expectations The PSW will also help ensure that Carla's established routines continue when family members are not present.

This includes:

Supporting medication routines through reminders and observation Being attentive to noticeable changes in Carla's mood, behavior, health, or daily functioning Reporting concerns or noticeable changes to the family Assisting with Urgent Care or virtual medical appointments when requested Helping maintain a clean and organized home environment Helping ensure meals are prepared and available for Carla during the week Encouraging Carla to participate in tasks she is able to complete independently Supporting community integration and social engagement Once per month, the PSW will also assist Carla with a weekend or evening community outing. Outings may include a nail appointment, outdoor event, library event, gym, shopping, or another appropriate community activity. The goal is to provide consistent support while encouraging Carla's independence, rather than completing every task for her. Important Notes On Mondays, Carla is allowed to stay home and does not require PSW support. On Saturdays, Carla is usually with her partner and does not require PSW support. During the evenings, Carla is familiar with her evening routine. Evening support is intended to remain low pressure as long as meals have been prepared and portioned into containers. Grocery and monthly hygiene items can be purchased through home delivery services by the family. This allows the PSW to use scheduled work hours for direct support and household responsibilities. The family will provide a $100 monthly stipend to support reasonable availability outside of scheduled hours when additional assistance is needed. Mileage reimbursement is available for approved outings or emergency medical appointments. The home has indoor and outdoor security cameras. Carla responds best to soft, patient, respectful, and understanding communication. The family expects all support to be provided in a respectful, professional, and person-centered manner.
