This energetic role offers flexible hours and the opportunity to provide vital assistance in various settings, including home health, assisted living, and memory care facilities. Duties Assist clients with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support using patient transfer methods like Hoyer lifts and manual handling techniques. Monitor patients' vital signs and observe for any changes in health status, documenting findings accurately while adhering to HIPAA regulations. Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation. Support clients with medication administration following care plans and ensuring safety protocols are maintained. Provide companionship and emotional support, engaging clients in social activities or recreational therapies suited to their interests. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning patient areas and assisting with laundry. Offer transportation assistance for appointments or community outings, ensuring safe patient transfers and mobility support. Experience Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred. Demonstrated ability to observe patients closely, monitor vital signs, and recognize signs of distress or health deterioration. Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with neurodevelopmental or developmental disabilities enhances your suitability for this role. Experience with patient transfer methods such as Hoyer lifts or other manual handling techniques is advantageous. Familiarity with creating and following care plans, administering medications safely, and providing personal care assistance is essential. Certification in CPR and First Aid is required; additional certifications related to home & community care or social work are a plus.
