Job Description

Seeking a motivated individual who is interested in working as a full-time caregiver with some office work mixed in. Great experience to add to any resume in the healthcare industry. Healthcare Plus has an immediate opening in its Sterling office for a Field Supervisor in Whiteside, Lee, Ogle, and Carroll Counties. Previous CNA experience is a plus. Training is provided, and all resumes will be considered. No previous experience is required. This is a full-time position that will include both working in the field as a caregiver and administrative duties in the office.
General Requirements:
GED or High School Diploma Social Security card or Birth Certificate Driver's License Clean driving record with car insurance Provide support to the team by covering unexpected call off / sick days from our field workers Start initial or interim cases as needed by the branch Provide one-on-one client care to clients who are not able to look after themselves Assists clients with activities of daily living as directed in their Plan of Care. Assists clients by providing housekeeping and laundry services, shopping for food and other household needs, preparing and serving meals and snacks, and running errands Assists clients by providing personal care such as bathing, dressing, grooming, and diaper changes Assists clients to care for themselves by monitoring the use of a cane, walker, wheelchair, special utensils to eat, and equipment for personal hygiene Monitor nutrition, cleanliness, and housekeeping Record the client's activities and changes by making entries in the client tasks report and notifying the supervisor of any unusual conditions Contact the case supervisor immediately for any refused or undelivered service Maintain a safe, secure, and healthy client environment by following Universal Standards Precautions, maintaining security procedures, and following the special instructions standards Ability to lift up to 35 pounds without assistance Adheres to professional standards, home care policies and procedures, as well as federal, state, and local requirements Provide client transportation to and from doctor appointments as needed Be willing to work in a household where the client has one or more of the following preferences: pets (dogs, cats, etc.), smoking, and hoarding living conditions In the event that the field supervisor doesn't receive a field assignment for a day, they must help in the office by filing paperwork, making calls, documenting notes in the Portal, organizing presentation materials, or any tasks assigned by the Lead Supervisor.
