Job Description
We are seeking a dedicated and compassionate Experienced In-Home Caregiver to provide exceptional care and support to clients in the Decatur, il. and surrounding areas. This vital role involves assisting individuals with a variety of needs to promote their independence, safety, and well-being within their homes. The ideal candidate will possess a strong background in home health care, senior care, or assisted living environments,with a focus on delivering personalized, respectful, and professional support. This position offers an opportunity to make a meaningful difference in the lives of those requiring long-term or short-term care. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor patients' vital signs and observe for any changes in health status or behavior, documenting findings accurately for care plans. Administer medication reminders following prescribed instructions while adhering to HIPAA regulations to ensure patient confidentiality. Prepare nutritious meals and or meal prep tailored to individual dietary needs and assist with feeding as required. Support patients with cognitive impairments such as dementia or Alzheimer's disease through memory care techniques and personalized engagement strategies. Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organization. Provide transportation assistance for medical appointments, social activities, or errands to promote community involvement. Assist with the implementation of individualized care plans developed by healthcare professionals. Support patients during hospice or palliative care situations with compassion and professionalism. Communicate effectively with clients' families and healthcare teams to ensure coordinated care delivery. Skills Proven experience working with individuals in home health settings, assisted living facilities, nursing homes, or group homes. Knowledge of patient observation techniques and ability to monitor vital signs accurately. Excellent communication skills for effective client interaction and documentation. Competence in meal preparation tailored to specific dietary restrictions or health conditions. Knowledge of HIPAA compliance standards to maintain confidentiality of patient information. Ability to adapt quickly to different client needs within diverse home environments.
