Responsibilities:

Provide high-quality, personalized care to clients in their homes, focusing on their physical, emotional, and social well-being Develop and implement personalized care plans in collaboration with clients, family members, and healthcare professionals Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming Prepare meals, manage medication, and provide transportation (as needed) Maintain accurate records and communicate regularly with the care team Strengthen relationships with clients and their families, providing emotional support and companionship
