Job Description

Mission driven social service agency is seeking a hardworking, self-motivated, Service Provider to provide support services to individuals with developmental disabilities. This position is primarily responsible for supporting individuals that live in their own homes to become valued members of their community through skill building and the development of natural supports.
Summary:
Support may occur in a variety of settings and may include providing personal care, companionship, assistance with daily living activities, assisting in communication and self expression, and ultimately supporting children and adults with developmental disabilities in a variety of home and community settings 15-20 hours; Flexible schedule; evenings required Essential Duties and Responsibilities Provide direct assistance and support to individuals with their personal care needs. Assist with grocery shopping and meal preparation. Coordinate leisure and educational activities. Assist with light housekeeping (e.g., laundry, dishes, vacuuming) as needed. Monitor and address medical needs of those involved in services. Coordinate with day services, employment, education activities, counseling services, volunteer activities, community involvements and/or any other services. Qualifications Experience working with children/adults with disabilities preferred. Valid Drivers License Proof of Auto Insurance and Vehicle Registration is required. Must have reliable transportation. Ability to communicate clearly in a variety of situations essential. 18 years of age or older
