We are looking for a reliable candidate to seniors living independently in their home. The care is person based to fit the daily needs if the individuals care plan. We strive to provide a safe and responsible care plan for clients and caregivers. Major Duties Assist clients with personal care which includes bathing, dressing, medication reminders, transportation, transfer services, homemaking services, meals, etc. Maintain open communication between families, staff and health care professionals regarding client medication and emotional condition, Document and report any changes in client's health status Ensure client's safety and well-being Staff member must make follow documentation standards providing direct care supervision of care to participant must make at least one entry on each day of service. Protects the home care agency by adhering to professional standards, home care policies and procedures, federal, state, and local requirements Provide companionship to clients which may include but is not limited to accompanying client on walks, trips to social/recreational activities, assistance with hobbies, etc. Adhere to the agency's mission, vision and values

Pay:

$16.00 - $17.50 per hour
