In this vital role, you will provide exceptional personal care and support to individuals in various care settings, including home health, assisted living, nursing homes, and group homes. Your positive attitude and proactive approach will help enhance the quality of life for those you serve. Duties Assist residents or clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using proper patient transfer methods like Hoyer lifts and manual handling techniques Monitor patients' vital signs, observe changes in health status, and report findings to healthcare professionals promptly Administer medications accurately following care plans while adhering to HIPAA regulations to ensure patient confidentiality Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed Support patients with cognitive impairments including Alzheimer's disease, dementia, or neurodevelopmental disorders through specialized memory care and dementia care techniques Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning patient areas and ensuring infection control standards are met Document patient progress, update care plans regularly, and communicate effectively with clients, families, and healthcare teams about resident's rights and care needs Experience Minimum of one year of caregiving experience in settings such as group homes, assisted living facilities, nursing homes, or home health environments Proven experience working with individuals with disabilities, developmental or intellectual disabilities support, autism spectrum disorder, or neurodevelopmental disorders Familiarity with patient observation techniques, medication administration, vital signs monitoring, and first aid/CPR certification Hands-on experience in providing personal transportation assistance and assisting clients with mobility devices like Hoyer lifts Knowledge of HIPAA compliance standards and best practices for home & community care environments Prior involvement in creating or following detailed care plans tailored to individual needs in geriatrics or hospice care settings Ability to perform heavy lifting safely when required for patient transfers or manual handling tasks Join us if you're passionate about making a positive impact through caring for others!
