This role offers an opportunity to make a meaningful difference in the lives of clients by promoting independence, safety, and well-being. The ideal candidate will be energetic, detail-oriented, and committed to delivering high-quality care in various settings including home health, assisted living, or long-term care facilities. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and personal hygiene Support patients with mobility and transfer methods using equipment like Hoyer lifts and patient transfer techniques Monitor vital signs and observe patient conditions to ensure safety and well-being Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation Administer medications accurately following care plans while adhering to HIPAA regulations for patient privacy Provide companionship and emotional support to reduce feelings of isolation or anxiety Assist with light housekeeping tasks including cleaning, laundry, and maintaining a safe environment Support clients during medical appointments or community outings using personal transportation assistance Follow established care plans for residents with dementia, Alzheimer's, autism, or other neurodevelopmental disorders Ensure proper manual handling techniques during patient transfers and heavy lifting when necessary Respond promptly to emergency situations with first aid or CPR as required Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments Strong patient observation skills with the ability to monitor vital signs and detect changes in condition Knowledge of dementia care, memory care, Alzheimer's care, and hospice care practices Familiarity with HIPAA regulations to ensure confidentiality and privacy of patient information Ability to perform patient personal care assistance safely and effectively Skilled in medication administration, first aid, CPR, and emergency response procedures Experience working with individuals with disabilities or neurodevelopmental disorders such as autism or intellectual disabilities support Competence in using patient transfer methods including Hoyer lifts and other manual handling equipment Capable of meal preparation tailored to specific dietary restrictions or health needs Strong communication skills for effective client interaction and team collaboration Ability to handle heavy lifting safely while maintaining proper body mechanics Knowledge of home & community care practices along with long-term care experience is highly desirable Join us if you're ready to bring energy, compassion, and professionalism into a rewarding caregiving role.
