Job Description

Join Sevita as a Caregiver/Home Health Aide and support individuals of all ages and abilities in their homes and communities. In this hands-on role, you'll assist with personal care, mobility, meals, and daily living activities while promoting independence, dignity, and safety. You'll provide companionship, support community outings, and follow individualized care plans, documenting observations and reporting changes. Responsibilities Provide compassionate in-home personal care, including bathing, grooming, dressing, and toileting assistance Support safe mobility, transfers, and positioning; use assistive devices as directed Assist with meal planning, preparation, and feeding according to dietary needs Monitor and report changes in clients' physical or emotional condition to supervisors Remind clients to take medications as directed by care plans (no medication administration unless certified) Offer companionship, engage clients in meaningful daily activities, and support community outings Perform light housekeeping and maintain a clean, safe home environment Follow individualized care plans, documentation standards, and company policies Collaborate with nurses, therapists, and families to support person-centered goals Participate in ongoing training and development to enhance caregiving skills Required Skills Personal care assistance (bathing, grooming, toileting, dressing) Safe transfer and mobility support (gait belts, walkers, wheelchairs) Basic vital signs observation and reporting Meal preparation and safe food handling Infection control and standard precautions Documentation and basic electronic or paper charting CPR and First Aid skills Behavioral support and de-escalation basics Time management and task prioritization Safe driving and client transport (where required)
