The Attendant Care Associate is responsible for providing and assisting participants with activities of daily living, restorative and supportive care. Assists with activities within the home to maintain the home environment. Assists in monitoring and reporting the health status and providing a safe environment for the participant. Position Responsibilities (reports changes in participant's condition and needs to the Home Care Director or scheduling coordinator): Light housekeeping Meal Preparation Transport or escort for essential errands and medical appointment as needed Lifting/general ambulation of clients Hygienic care, such as grooming & bathing services Completes appropriate documentation of services provided following Horizon program policy Follows all Horizon program policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines Maintains the confidentiality of all company procedures, results, and information about participants, clients or families Participates in continuing education classes and any required staff and training meetings.
