Job Description
Overview Join our dedicated team as a Direct Support Worker (DSW), where your compassionate care and proactive support will make a meaningful difference in the lives of individuals with diverse needs. In this vital role, you will provide personalized assistance, promote independence, and ensure safety in various settings such as residential facilities, community homes, and home care environments. Responsibilities Assist individuals with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes to healthcare professionals promptly Administer medications following care plans and HIPAA regulations to ensure safety and compliance Prepare nutritious meals tailored to dietary needs while maintaining cleanliness and sanitation standards in the kitchen area Support individuals with developmental disabilities, autism, dementia, Alzheimer's, or memory care needs through engaging activities and compassionate supervision Maintain detailed documentation of care provided, incident reports, and updates to care plans in accordance with healthcare regulations Engage in light housekeeping duties such as cleaning resident areas to promote a safe and hygienic environment Experience Prior experience working in assisted living, nursing homes, or home & community care environments preferred Knowledge of patient observation techniques, medication reminders, and vital signs monitoring required Familiarity with working with individuals with disabilities, including developmental disabilities, autism, or dementia care highly desirable Certification in CPR and First Aid is strongly recommended; additional training in hospice or memory care is a plus Experience with patient monitoring tools like Hoyer lifts or resident's equipment is advantageous Demonstrated ability to perform heavy lifting safely and assist with mobility support as needed Strong understanding of HIPAA regulations to ensure confidentiality and privacy of patient information Join us in providing exceptional care that empowers individuals to live their fullest lives.
