DSW (Direct Support Worker)
DIRECT SUPPORT WORKER
(DSW) (DSW) will assist clients with intellectual disabilities on daily living activities such cooking, personal hygiene, transportation, community recreational activities and accomplishment of goal and objectives. Also, to work closely with the QMRP to help develop a person center plan for each the clients. Document progress on individual program plans, social activities, seizure log, medication log and family contact. Provide direct training and supervision to the resident in cooking, cleaning, laundry, and socializing, personal grooming, and money management. Train resident on safety issues in the residence, in traveling, in stranger awareness, and telephone conversation. Document incidents correctly on the appropriate forms. Supervise self-administration of medication and properly document in medication log.Job Type:
