Established in 2011, our organization emphasizes strong leadership and a committed team of healthcare professionals to support patients in remaining comfortably within their homes and communities. Overview We are seeking a dedicated and skilled Physical Therapist or Physical Therapist Assistant (PT/PTA) for our Home Care team. This role offers an exciting opportunity to deliver comprehensive rehabilitative services directly to patients in their homes, promoting recovery, independence, and improved quality of life. The ideal candidate will possess a strong foundation in patient care, physiotherapy techniques, and home health practices. Our main office is located in Methuen. Duties Assess patients' physical conditions and develop personalized care plans tailored to their specific needs, including discharge planning and management of acute care cases. Provide physiotherapy interventions such as therapeutic exercises, manual therapy, and trigger point therapy to improve mobility and reduce pain. Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care across inpatient, outpatient, hospital, and community settings. Educate patients and their families on physiologic functions, injury prevention, and self-care strategies to support ongoing recovery. Maintain accurate documentation of patient progress using electronic medical record (EMR) systems while ensuring compliance with healthcare regulations. Support patients with diverse needs including pediatrics, geriatrics, orthopedics, and post-surgical rehabilitation within home health or outpatient clinics. Participate in care plan reviews and discharge planning processes to ensure seamless transitions between care settings. Requirements Valid licensure as a Physical Therapist or Physical Therapist Assistant with relevant state certification. Prior experience in home health care or community-based physiotherapy is highly desirable. Strong knowledge of anatomy, physiology, and medical terminology essential for effective patient assessment and treatment planning. Experience working with inpatient hospital cases, outpatient clinics, or skilled nursing facilities preferred. Familiarity with EMR systems for documentation and patient record management. Demonstrated ability to perform physiotherapy techniques such as trigger point therapy and orthopedic rehabilitation procedures. Excellent communication skills to effectively interact with patients of all ages—including pediatrics—and multidisciplinary teams. Knowledge of discharge planning processes and experience managing care plans across various healthcare environments is advantageous.
