Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Support patients with medication administration, vital signs monitoring, and adherence to care plans tailored to individual needs. Provide companionship and emotional support while observing patient conditions closely to detect changes or concerns promptly. Prepare nutritious meals aligned with dietary requirements and assist with feeding as needed. Facilitate safe patient transfers using manual handling techniques or equipment such as Hoyer lifts, ensuring safety for both client and caregiver. Maintain accurate documentation of patient care activities, observations, and communication with healthcare professionals while adhering to HIPAA regulations. Offer transportation assistance for medical appointments, community outings, or errands to promote social engagement and independence. Skills Proven experience working with individuals with developmental disabilities, neurodevelopmental disorders, or aging populations in settings such as group homes, assisted living facilities, nursing homes, or home health environments. Strong knowledge of patient observation techniques, dementia care (including Alzheimer's and memory care), hospice care, and managing residents' 's needs. Skilled in performing patient monitoring tasks such as vital signs measurement, medication administration, and recognizing emergency signs requiring first aid or CPR intervention. Familiarity with care tools like Hoyer lifts, patient transfer methods, and manual handling procedures to ensure safe mobility assistance. Excellent communication skills for effective client interaction and collaboration with healthcare teams or social workers. Ability to perform light housekeeping duties including cleaning to maintain a safe and hygienic environment for clients. Experience working with individuals with autism or other intellectual disabilities; understanding of HIPAA compliance and home & community care standards.
