Duties Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and personal hygiene to ensure comfort and dignity. Support clients with mobility and transfers using proper patient transfer methods and equipment like Hoyer lifts to promote safety. Monitor vital signs, observe patient conditions, and report changes promptly to healthcare professionals or family members. Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking to promote healthy living. Administer medication following care plans and ensure compliance with HIPAA regulations for patient privacy. Provide companionship and emotional support, engaging clients in meaningful activities to enhance mental well-being. Maintain a clean, safe environment through light housekeeping, cleaning routines, and organization of client spaces. Support clients during medical appointments or community outings using personal transportation assistance or client transportation methods. Follow all safety protocols including first aid, CPR certification requirements, manual handling procedures, and emergency response protocols such as hospice care or dementia care situations. Document care activities accurately in compliance with healthcare standards and communicate effectively with clients' families and healthcare teams. Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Pca or hha certification at minimum. Strong patient observation skills with the ability to recognize changes in condition or behavior. Knowledge of Alzheimer's care, memory care techniques, dementia care strategies, and neurodevelopmental disorders support. Skilled in assisting with ADLs, patient transfer methods—including Hoyer lifts—and manual handling techniques. Familiarity with medication administration, vital signs monitoring, patient monitoring systems, and care plan implementation. Excellent communication skills for client interaction, family updates, and team collaboration. Ability to perform heavy lifting safely while adhering to manual handling standards; experience working with individuals with disabilities or developmental delays is highly valued. Experience providing home & community care services across diverse populations including geriatrics and intellectual disabilities support. Certifications such as hha or pca ,first aid, CPR, HIPAA compliance training are required; additional certifications related to hospice or long-term care are advantageous but unnecessary.
