Job Description
The client is currently at a rehab center, and the caregiver will visit on Saturdays and Sundays until discharge. During this time, the caregiver will accompany her, and the hours of care will gradually increase as needed. This arrangement ensures that the client receives consistent support and companionship during her recovery process. The caregiver's presence will not only assist with daily activities but also provide emotional support, which is crucial for the client's well-being. As the discharge date approaches, a reassessment of the care plan will be conducted to ensure that all needs are met effectively. This proactive approach will help facilitate a smooth transition back home.
