Responsibilities Deliver personalized resident and patient care, including assistance with activities of daily living such as bathing, dressing, grooming, and mobility support. Administer medications accurately following prescribed schedules and document all medication administration in medical records using Electronic Health Record (EHR) systems. Monitor vital signs like blood pressure, pulse, respiration, and temperature; report any abnormalities promptly to healthcare professionals. Assist with specialized medical procedures such as tube feeding, catheter care, and wound management while maintaining strict adherence to HIPAA regulations and patient confidentiality. Support residents with dementia or memory care needs by implementing tailored activities and ensuring a safe environment aligned with Alzheimer's and dementia care best practices. Maintain detailed medical documentation using EMR (Electronic Medical Record) systems like Epic or Cerner; ensure compliance with state healthcare regulations. Perform manual handling tasks safely during heavy lifting or transfer of residents/patients to prevent injury for both staff and individuals under care. Respond swiftly to emergencies by providing first aid or CPR when necessary; stay current with certifications in first aid and basic life support (BLS). Collaborate effectively with interdisciplinary teams to coordinate comprehensive patient care plans that meet individual needs. Uphold high standards of senior care, hospice support, and home & community care services by demonstrating excellent caregiver communication skills. Experience Proven experience working in assisted living facilities, nursing homes, hospitals, or home care environments. Familiarity with medical terminology, physiology, anatomy knowledge, and medical documentation practices. Prior experience caring for geriatric patients or individuals with disabilities is highly desirable. Knowledge of state Board of Nursing requirements and healthcare regulations applicable to caregiving roles. Hands-on experience with medical equipment such as EHR systems (Epic, Cerner), vital sign monitors, tube feeding devices, and other medical tools. Demonstrated ability to perform heavy lifting safely while maintaining resident safety standards. Strong caregiving skills complemented by excellent communication abilities to effectively interact with patients, families, and healthcare teams.
