Support medication administration and manage medication schedules following prescribed instructions. Facilitate patient transfer methods using equipment like Hoyer lifts and manual handling techniques to ensure safety during mobility assistance. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Provide companionship and emotional support, fostering positive communication and engagement with clients. Maintain a clean and safe environment through routine cleaning tasks and adherence to infection control protocols. Document patient observations, care activities, and incident reports accurately to ensure seamless communication within the care team. Assist with transportation needs for appointments or community outings while ensuring client safety at all times. Support clients with cognitive impairments such as dementia or Alzheimer's disease by implementing specialized memory care strategies. Requirements Proven experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred. Knowledge of patient observation techniques, dementia care, Alzheimer's care, and memory care practices. Familiarity with patient monitoring tools such as vital signs measurement and medication administration procedures. Ability to perform manual handling tasks including heavy lifting and patient transfers using Hoyer lifts or other assistive devices. Certification in CPR and First Aid is required; additional certifications in HIPAA compliance or hospice care are advantageous. Experience working with individuals with developmental disabilities, neurodevelopmental disorders, autism spectrum disorder (ASD), or intellectual disabilities is beneficial. Strong communication skills for effective client interaction and documentation; compassionate caregiving attitude essential. Physical stamina to stand, walk, lift heavy objects safely, and perform caregiving duties over extended periods. Valid driver's license or access to reliable transportation is a plus for assisting with community outings or appointments.
