In this vital role, you will provide personalized assistance to individuals with diverse needs, including elderly patients, those with developmental disabilities, and individuals requiring memory or hospice care. Duties Assist clients with activities of daily living (ADLs), such as bathing, dressing, grooming, and toileting, ensuring their dignity and comfort are maintained at all times Monitor patient health by observing vital signs, administering medications, and documenting medical information accurately using Electronic Health Record (EHR) systems Support patients with mobility using transfer methods such as Hoyer lifts and ensure safe patient transfers to prevent falls or injuries Provide specialized dementia and Alzheimer's care, including memory care strategies and creating a safe environment for clients with cognitive impairments Assist with medical procedures like catheter care, wound care, and basic life support (BLS), including CPR when necessary Prepare nutritious meals aligned with dietary restrictions and assist with feeding as needed while maintaining food safety standards Maintain strict confidentiality of patient records in compliance with HIPAA regulations and home healthcare laws Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred Demonstrated experience caring for geriatric patients or individuals with disabilities, including dementia or Alzheimer's disease Knowledge of medical terminology, medication administration protocols, and vital signs monitoring Hands-on experience with home health care assistance, hospice care, or long-term care environments Familiarity with EMR/EHR systems for documentation and client communication Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts Strong understanding of home & community care regulations and best practices for patient safety and confidentiality Join us to provide compassionate caregiving that transforms lives!
