Personal Care & Community Support Worker Bay City, MI$22.00-$22.40 per hour Part-Time |25.5 Scheduled Hours + Optional Extra Shifts Looking for a dependable person to provide direct, one-on-one support to an adult with physical and developmental disabilities in a private home and in the community. This position is part of a Self-Determination Community Living Supports (CLS) program. You will work with one individual rather than caring for multiple clients in a facility. The job combines hands-on personal care with mobility assistance, community activities, and support toward Person-Centered Plan (PCP) goals. Work Schedule The regular schedule is: Monday through Friday3:10 PM-7:10 PM Monday Night10:30 PM-4:00 AM Total regular schedule: 25.5 hours per week Applicants must be available for the regular schedule listed above. Opportunities for Additional Hours Employees may also have opportunities to pick up open shifts when additional coverage is needed. Depending on staffing and client needs, these may include daytime, evening, weekend, school-break, and summer hours. Additional hours are optional when offered and are not guaranteed. What You'll Be Doing This is an active, hands-on support position.

Responsibilities include:

Working directly on Person-Centered Plan (PCP) goals Supporting community outings and activities Encouraging independence and participation in daily activities Assisting with standing programs and adaptive equipment Supporting the individual in working toward using a standard gait trainer Performing Hoyer lift transfers Providing mobility, positioning, and hands-on physical assistance Assisting with personal care, hygiene, toileting, and brief changes Providing personal care during community outings when needed Assisting with meals and self-feeding Completing required care documentation Maintaining a safe and respectful environment This Is a Physically Active Position The individual requires hands-on physical assistance. Job duties include lifting and assisting with Hoyer transfers, repositioning, bending, stooping, kneeling, assisting with adaptive equipment, and pushing or maneuvering mobility equipment. Applicants must be able to safely perform the essential functions of the position, with or without reasonable accommodation. What We're Looking For Dependable and consistent attendance Reliable transportation Comfortable working one-on-one in a private home and in the community Comfortable providing hands-on personal care, including toileting and brief changes Willing to participate in community activities and PCP goals Able to follow established care instructions and complete documentation Able to safely perform the physical duties of the position, including lifting and hands-on transfers Previous experience as a Caregiver, DSP, CLS Worker, CNA, Home Health Aide, Personal Care Assistant, or similar role is preferred but not required.
